Job Description:

Phillips County Health Systems is currently seeking a Community Health Worker. Career Summary: The community health worker (CHW) will support and coach at-risk patients to manage their own health and assist them in navigating the health care and social service systems. CHWs work in safety net clinics, hospital systems, and community-based agencies. Community Health Workers (CHWs) bridge the gap between health care providers and people in need of care. They serve as a link between the patient and the health or social service agencies. Key Responsibilities: - Serve as a liaison between patients, healthcare providers, and community resources. - Support and coach at-risk patients in managing their health and wellness goals. - Conduct home visits and accompany patients to medical and social service appointments as needed. - Assist patients with navigating healthcare, behavioral health, and social service systems. - Help patients access transportation, medications, financial assistance programs, interpreters, and other support services. - Collaborate with patients to develop and monitor individualized health goals and action plans. - Provide health education and encourage adherence to treatment plans and prescribed medications. - Review referrals and patient needs assessments to determine eligibility and appropriate services. - Maintain regular communication with providers and referral sources regarding patient progress. - Complete required documentation, data tracking, and reporting accurately and in a timely manner. - Participate in required trainings, meetings, and program development activities. - Build strong relationships with clinic staff, community partners, and patients. Qualifications: - High school diploma or equivalent required. - Strong computer skills, including proficiency with Microsoft Office. - Excellent communication, interpersonal, and relationship-building skills. - Ability to work independently and as part of a team. - Strong organizational, time management, and multitasking abilities. - Experience working with diverse, underserved, or vulnerable populations preferred. - Knowledge of healthcare and social service systems preferred. - Ability to motivate, coach, and support individuals in achieving personal health goals. - Bilingual abilities are preferred. - Reliable transportation, valid driver's license, and current vehicle insurance required.
